Please bring along your Insurance information and I.D.

We will request you to sign an initial contract for services and fill out a mental health questionnaire. ​ Please arrive about 15-20 minutes early for the first visit to be able to complete these things in time. You can choose to save some time by printing these forms at home to fill them and bring them along.

A list of any current medications or medication bottles and results of any recent lab work will be useful.​

Expect to spend about 40-50 minutes with the Psychiatrist for the first visit, who will take a detailed history and discuss the initial treatment plan with you.
